Laryngeal Paralysis

The larynx is a set of cartilages at the top of the windpipe whose job is to swing open on every breath in and to snap shut every time the cat swallows. Both functions depend on tiny muscles controlled by the recurrent laryngeal nerve, and when that nerve fails the cartilages sag inward, sitting in the airway like a half-closed door.
The first thing most owners notice is not the breathing but the voice. A cat that has always had a small chirpy meow suddenly sounds hoarse, or gravelly, or produces a strange deep note quite unlike itself — the videos of cats with startlingly deep "motorbike" meows that circulate online are, in many cases, cats with laryngeal paralysis. Because the change is gradual and often charming rather than alarming, it is easy to dismiss.
The breathing problem develops later and follows a nasty logic. The narrowed opening makes air move faster through it, which drags the flaccid cartilages further inward, which narrows the airway more. Anything that makes the cat breathe harder — heat, excitement, exertion, a stressful car journey — can tip a manageable partial obstruction into a genuine crisis. Cats in this state overheat rapidly and can become severely oxygen-deprived within minutes. This is why the first-aid priorities are calm, cool and quiet, and why sedation given by a vet is not merely for comfort but is part of breaking the cycle.
Because laryngeal paralysis in cats so often reflects something else going on, diagnosis involves more than looking at the larynx. The vet will typically sedate the cat lightly and watch the arytenoid cartilages during breathing to confirm they are not abducting, then look for a cause: chest and neck imaging for masses, a careful history for recent thyroid surgery or neck trauma, and a neurological examination for signs of wider nerve or muscle disease.
Surgery is effective but is a genuine trade-off. Tying one side of the larynx permanently open removes the obstruction, but it also removes part of the mechanism that keeps food and water out of the lungs. Owners of tie-back cats are taught to feed meatballs of wet food rather than dry kibble, to avoid swimming and dusty environments, and to treat any cough or fever as a possible aspiration pneumonia. For a cat that has been struggling to breathe, most families find it a trade worth making.
🔍 Causes
In cats, laryngeal paralysis is most often acquired rather than inherited. Damage to the recurrent laryngeal nerve is the usual mechanism — from trauma to the neck, from surgery in the region (thyroidectomy for hyperthyroidism is a recognised cause), from a mass in the neck or chest pressing on the nerve, or as part of a generalised neuromuscular disease. It can also follow certain toxicities and, occasionally, no cause is ever found. Unlike the dog, where the condition is a well-known problem of older large breeds, feline cases are uncommon enough that a thorough search for an underlying cause is always warranted.

There is no direct prevention, but avoiding neck trauma helps: use a harness rather than a collar for cats that pull on a lead, and use breakaway collars. Keep cats at a lean body weight and out of heat and humidity, since obesity and high temperatures markedly worsen any partial airway obstruction. Follow up carefully after any surgery in the neck region and report voice changes to your vet.

Emergency cases need sedation, oxygen and cooling — panicking makes the airway swell and worsens the obstruction, so calm sedation is genuinely therapeutic. Mild cases in cats can be managed conservatively with weight control, harnesses rather than collars, avoidance of heat and exertion, and treatment of any underlying disease. The surgical option is a unilateral arytenoid lateralisation, or "tie-back", which permanently holds one side of the larynx open. It relieves the obstruction reliably but leaves the airway less protected, so aspiration pneumonia becomes a lifelong risk that owners must understand before consenting.
