Pyothorax (Pus in the Chest)
Infected fluid accumulating in the chest cavity, compressing the lungs. It is a true emergency, but with aggressive drainage most cats survive.
⚠️ Common symptoms
The chest cavity holds only a tiny amount of lubricating fluid in health, just enough for the lungs to glide against the chest wall. There is no spare room. When infection fills that potential space with pus — sometimes hundreds of millilitres of it — the lungs are compressed from outside and simply cannot inflate. The cat is not, strictly speaking, unable to move air; it is unable to make room for it.
That mechanical reality explains why the first-line treatment is so blunt and so effective. Draining fluid with a needle relieves the compression within minutes, and a cat that arrived gasping can be visibly comfortable before the diagnostic work-up has even finished. It also explains why these cats are so fragile to handle: the reserve that lets a healthy cat tolerate being held for an X-ray has already been spent.
Diagnosis is usually quick. Muffled heart sounds and a chest that sounds dull on percussion point the way, ultrasound confirms free fluid, and a sample examined under the microscope settles it immediately — degenerate white cells, and often visible bacteria, sometimes in the filamentous chains characteristic of the anaerobes that dominate feline cases. Fluid always goes for culture, including anaerobic culture, because the antibiotic choice for the following six weeks depends on it.
What separates good outcomes from poor ones is rarely the antibiotic. It is the drainage. Pus is thick, it walls itself off into pockets, and antibiotics penetrate it poorly. Cats managed with indwelling drains and repeated lavage over several days do substantially better than cats treated with intermittent needle drainage and drugs alone, and the reported survival figures for aggressively managed cases — commonly quoted in the region of eighty percent — are far better than the disease's frightening presentation would suggest.
It is a demanding week. The cat needs hospitalisation, twice-daily or more frequent flushing, careful nursing, pain relief and coaxing to eat, and the bill reflects that. But this is one of the emergencies where the investment is genuinely repaid: a cat that survives the first few days and completes its antibiotic course usually recovers completely, with no long-term consequences and no ongoing medication. The tragedy of pyothorax is almost always the cat that arrived too late, not the cat that was treated too hard.
🔍 Causes
The commonest route in cats is thought to be from the mouth into the chest, with the same anaerobic and Pasteurella species that populate feline saliva and cat-bite abscesses turning up in chest fluid. Penetrating bite wounds to the chest wall are a well-recognised cause, as is inhaled plant material such as a grass awn migrating into the lung, extension of pneumonia, a ruptured oesophagus or airway, and spread through the bloodstream from infection elsewhere. Young adult cats and outdoor cats in multi-cat households are over-represented, which fits the bite-wound and fighting picture, although a substantial proportion of cases never yield a definitive source.

Neutering reduces roaming and fighting more effectively than any other single measure, and fighting is the behaviour most closely tied to this disease. Any cat-bite wound should be treated properly and early rather than waited out — a course of antibiotics started within a day or two prevents an enormous amount of downstream trouble, and wounds over the chest deserve particular respect. Keeping cats in overnight and reducing tension between cats in multi-cat households cuts the number of fights. Beyond that, vaccination and prompt treatment of respiratory infection reduce the chance of pneumonia extending into the chest cavity.

Cats presenting in respiratory distress are stabilised with oxygen and minimal handling before anything else — a cat struggling to breathe can be tipped over the edge by restraint alone. Fluid is then drained from the chest by needle, which often produces dramatic and immediate relief. Definitive treatment means placing indwelling chest drains, usually on both sides, and flushing the chest cavity with warmed sterile saline repeatedly over several days until the fluid returns clear. Broad-spectrum intravenous antibiotics start immediately and are refined once culture results identify the organisms, with the total course typically running four to six weeks. Intravenous fluids, pain relief and nutritional support are all part of the picture, since these cats are usually dehydrated and have not eaten in days. Surgery is reserved for cats that fail to respond to drainage or that have a discrete abscess, a foreign body such as a grass awn, or a lung lobe that needs removing.
